Pet Services Master Client Agreement

Our Master Client Agreement must initially be completed and signed prior to receiving services from The Animal Tender Pet Resort; it provides information on the terms and conditions applicable to all pet boarding, pet grooming, doggie day care services and dog training and remains in effect for all current and future stays at the pet resort for all pets in your household.  We encourage you to read this agreement in its entirety before arriving at The Animal Tender Pet Resort.  Please print out and bring a signed copy of the entire agreement with you when your pet checks in.

Medical Intake Form 

The Medical Intake Form is to be filled out by the owner/agent of the dog. All medications that the dog will need while in the care of The Animal Tender should be written down with as much detail as possible. 

You only need to complete this form once, although we will ask you during subsequent visits if any information about your pet has changed since you initially completed the form.  In order to expedite your check-in time, please print out and bring a signed copy of this document with you for each pet at check-in time.

Personal Items Policy

Clean bedding will be provided for all pets boarding with us. We understand the desire to leave personal bedding, toys, treat, etc. If any personal items are left with the pet, they must be personalized with permanent magic marker and listed on our Personal Items Policy form. Only two toys per pet are allowed and The Animal Tender does NOT accept rawhide bones or any treats that may become indigestible or get lodged in the pet’s throat. In order to expedite your check-in time, please print out and bring a signed copy of this document with you for each pet at check-in time.
